How the scoring works

Each question maps to a specific ISO 9001:2015 requirement and is scored on whether the requirement is absent, informal, documented, or documented with objective evidence that it is actually being followed. That last distinction is what separates a binder from a working quality management system: registrars do not audit intent, they audit records. Clauses with the highest audit weight — internal audit, management review, corrective action, control of documented information, and control of externally provided processes — carry more weight in the tier calculation, because those are the areas where major nonconformities are most frequently raised.

Common gaps this assessment surfaces

• A quality policy that exists but has never been communicated to production personnel (clause 5.2.2).
• Risks recorded once during implementation and never reviewed against actual performance (clause 6.1).
• Competence records that list training attendance but no evaluation of effectiveness (clause 7.2).
• Supplier approval with no re-evaluation criteria or performance monitoring (clause 8.4.1).
• Nonconformances closed without root cause analysis or verification of effectiveness (clause 10.2).
• Internal audits covering only the quality department instead of every process in the QMS scope (clause 9.2).

What to do with the result

Foundational and Developing results usually mean the fastest path is building the documented information and registers first, then running a full internal audit cycle before booking Stage 1. Mature and Audit-Ready results generally mean the system exists but the evidence trail is thin in one or two clauses — that is a matter of weeks, not months. In either case, treat the clause-level breakdown as the agenda for your next management review, assign an owner to every gap, and set a target date before your audit window opens.

How long certification actually takes

For a single-site manufacturer of 10 to 150 people with no prior system, eight to sixteen weeks of focused work is realistic when procedures, registers, and the audit program are generated and maintained in one place. The schedule slips when documented information lives across spreadsheets, shared drives, and email, so every audit becomes an evidence hunt.
